A gender ninja is an androgynous-looking person, in other words, a person whose gender is indecipherable. One of the most famous examples of a gender ninja is Pat from the 1990s SNL sketches.
Whether it be long hair or a baby face, sometimes it's hard to tell if a person is male or female. The "ninja" part of the term conveys sneakiness or mysteriousness, which is when people don't know whether to label a person male or female.
