In general use, people use it when they are weary of someone's past decisions or experiences. It may be a bad childhood, sexual history, or any other negative or harmful experience.
In sports, especially fantasy sports, it is mostly used for describing a player who has some upside but also significant downside. It often is an injury that will likely decrease his production.
